DC has not revealed yet why and how he actually disappeared. Some say it's because: 1) He dies while coming out of the Speed Force. 2) He runs so fast that he disintegrates into the air. 3) He time travels and he is unable to time travel to his time because he loses his powers.
During The Flash's midseason finale, we learned that Nora has been conspiring with Eobard Thawne, aka the Reverse-Flash, behind her parents' backs. Even after she finds out that he's responsible for her grandmother's death, she continues to work with him.

As I suspected last week, it turns out that the dagger is currently being used to suppress Thawne's powers, so he sent Nora back to defeat Cicada because he wanted Team Flash to destroy the dagger in the past, which would then free him in the present.
Killing Oliver in the first episode, he said, was a decision made to circumvent the expectations of viewers. "We've kind of figured that the audience is expecting that Oliver will die in the climax in the fifth hour," Guggenheim said.
