How to Create a Survey
- Choose the right platform.
- Make the survey as short as possible.
- Don't ask "yes" or "no" questions.

Microsoft Forms is an Office 365 application for creating online surveys or quizzes. The survey owner is shown the responses and how popular specific answers are. There is also an option to access this data in Excel.

- Go into your list's settings page and click on “Save list as Template”
- Check off the “Include Content” option.
- Click the “Download a Copy” button (it'll generate a .stp file)
- Access the “List Templates” on your Site Settings page.
Microsoft Forms is an Office 365 application for creating online surveys or quizzes. There are also options for adding a new question to the survey. Question types range from multi-choice, to simple text boxes and ratings. Everything is straight forward, and simple to use.
An Excel Survey is a Web form, that you design to collect and store structured data, into an Excel spreadsheet. You have many options when it comes to Web surveys or forms.
New form
- Click Add new form.
- In the panel on the right, provide a name for your new form.
- Click Create.
- Microsoft Forms will open in a new tab. Learn how to create a form.
- When you're done creating your form, go back to your SharePoint Online page.
Create a form with Microsoft Forms
- In your web browser, go to forms.office.com.
- Sign in with your Office 365 school credentials, Office 365 work credentials, or Microsoft account (Hotmail, Live, or Outlook.com).
- Under My Forms, click New Form to begin creating your form.
- Enter a name for your form.
- Click Add Question to add a new question to the form.
Creating Survey in Sharepoint 2010
- Open your Sharepoint site.
- Click more option from site actions, and create Survey from it.
- When the Survey is created it prompts to create questions.
- After typing the question we can select the type of answer for the Question.
- We can specify a detailed answer from the options below.

Microsoft Forms, much like SurveyMonkey, allows you to create surveys in Office 365, and either share them to authenticated users within your organization, or with anonymous respondents anywhere.
Create a form with Microsoft Forms
- In your web browser, go to forms.office.com.
- Sign in with your Office 365 school credentials, Office 365 work credentials, or Microsoft account (Hotmail, Live, or Outlook.com).
- Under My Forms, click New Form to begin creating your form.
- Enter a name for your form.
- Click Add Question to add a new question to the form.
Go to Survey Settings >> Advanced Settings >> Set the Item-level Permissions as: Read access & Edit access to: Only their own. This will hide all responses in SharePoint survey. End-users will get "Contributor" access to respond SharePoint Surveys.
To create a Manual Data Entry collector:
- Go to the Collect Responses section of your survey.
- Choose Add Data Manually and click Next.
- Click Add New Response and fill out the survey.
- Make sure to click Done on the last page to record the survey response.
Allowing Multiple Responses
- Go to the Collect Responses section of your survey.
- Click the name of the collector to see the collector option.
- Click Multiple Responses.
- Select On, allow the survey to be taken more than once from the same device. Your changes save automatically.
To set a survey-wide limit, we suggest using only one collector for your survey. To turn on Response Limits: Go to the Collect Responses section of your survey. Once the maximum is reached, anyone who tries to access the survey will see your closed message.
However if you have a regular Gmail / Google Account, you have another option now to prevent multiple form submissions from the same user. If someone tries to fill the Google Form again, a warning message will be displayed saying ”You've already responded. You can only fill out this form once.
No, currently there's no option to have people enter multiple records, unfortunately. The only options are to have them fill out the whole form multiple times or repeat the questions within the form, but in the last case the repeated questions will always end up as extra columns in the spreadsheet instead of rows.

Resend an Invitation
From the Overview tab of the collector, click Send Reminder. In the top-right corner of the Send To field, click Edit Recipients. Remove any recipients you don't want to send the message to and click Save.In SurveyMonkey, you have a couple options:
- Turn on Multiple Responses using a Web Link Collector. You can also edit the Survey End Page to loop to the start of the survey so respondents can take the survey again, as many times as needed.
- Copy the question and add it to a page several times.
As long as the status of a collector says OPEN, people are able to access your survey through that collector and submit their responses. You can re-open a closed collector at any time by clicking the CLOSED status.
